Diabetes doesn’t come with a handbook, but DSMES is the next best thing. At Cullman Diabetes Center, you’ll learn what’s happening in your body, how to manage it day-to-day, and where to turn when things change.

Our accredited DSMES program follows the national standards for diabetes self-management. You’ll work with certified care and education specialists who help you build real knowledge, skills, and confidence to manage your health daily. That includes learning how to monitor blood glucose, make healthy eating choices, get active, and feel more in control of the aspects of diabetes that affect your life most.

It all starts with a one-on-one appointment to talk through your needs, goals, and questions. Working together with your care team and community, you’ll have everything you need to move forward confidently.

What to Expect at Your First Visit

Demystify Diabetes

We know how overwhelming diabetes can feel at first. That’s why your first visit is all about slowing things down and listening. You’ll meet with one of our educators or our registered dietitian, talk through your diagnosis, and set some practical goals for your next steps.

These trusted health professionals will help you understand your current plan, introduce goals like glucose monitoring, and talk through strategies for reducing risks and improving health outcomes over time.

We’ll explain what your insurance may require, whether you need a referral, and what programs or classes could help most. From there, we walk with you through your personalized care plan at your pace. Visit our resource center for more information!

Some insurance plans require a referral from your doctor, registered nurse, or another provider. But don’t worry—we’ll help you figure out what’s needed based on your coverage and goals. If you’re interested in getting started, just fill out the form or give us a call.

Our self-management education for diabetes includes one-on-one assessments, small group classes, and follow-up sessions focused on meal planning, glucose monitoring, healthy eating, and physical activity. Everything we teach aligns with the American Diabetes Association standards.

Type 2 diabetes may be put into remission for some people, meaning blood sugar levels return to a healthier range without the need for medication. This often involves changes in eating habits, physical activity, weight management, and support from a care team.

That said, remission doesn’t mean “cured,” and it’s not the right goal for everyone. Our focus is on helping you feel more confident living with diabetes, no matter what your numbers look like today. With the right tools, education, and support, better health is always possible.
